Attention Parents!
Community Health Clinic of Muscatine is coming to West Liberty on May 20 and 21st. They will be on-site and offer these services during the school day to students.
They offer:
School or Sports physicals
Immunizations
Vision screenings
Dental screenings
IF your child is entering preschool, kindergarten, 3rd, 7th, 9th, or 12th grade, there are state requirements to be met.
Preschool/Kindergarten: vision, dental, immunizations, physical
3rd: vision screening
7th: immunizations
9th: dental screening
12th: immunizations
Physicals and Sports Physicals are good for one year from the date of the physical.
CHC Muscatine can bill insurance, including Medicaid, for its services. These services are not free, so parents will either be billed or have insurance coverage applied. Additionally, a sliding scale payment option is available. All services will require a signed parental consent.

SI su hijo está ingresando a preescolar, jardín de infantes, 3, 7, 9 o 12 grado, existen requisitos estatales que debe cumplir.
Preescolar/Kindergarten: visión, dental, vacunas, examen físico
3 grado: examen de la vista
7 grado: vacunas
9 grado: examen dental
12 grado: vacunas
Los exámenes físicos y los exámenes físicos deportivos son válidos por un año a partir de la fecha del examen físico.
CHC Muscatine puede facturar al seguro, incluido Medicaid, por sus servicios. Estos servicios no son gratuitos, por lo que se facturará a los padres o se les aplicará la cobertura del seguro. Además, está disponible una opción de pago de escala móvil. Todos los servicios requerirán un consentimiento firmado de los padres.
