Incoming Freshmen:
Incoming 9th-grade students will need to turn in a Certificate or Dental Screening from the State of Iowa. This screening must be completed by a RDH (Registered Dental Hygienist) or DDS (Doctor of Dental Surgery).
Immunizations must be up to date, there are no requirements for 9th grade. 7th-grade requirements for a Tdap and MCV are enforced for students that are transferred in.
Students participating in sports must turn in a Sports Physical completed by a Licensed Medical Professional and a Concussion form filled out by parents.
Every year physicals with a Primary Care Provider are encouraged, as well as yearly vision exams.
Visits to the Dentist every 6 months are encouraged.

Incoming Seniors:
12th-grade students need proof of their 2nd Meningococcal vaccine (MCV4), this can be done with an Immunization record. Meningitis is a serious, life-threatening disease that is easily spread through close contact.
This needs to be given to the school nurse prior to starting the school year. If not done, students may not be able to start their senior year.
Please help me keep students compliant with the school. We are hoping for a great and healthy year.
